Job summary

Independent Living Systems (ILS) in Orlando, FL seeks a Supervisor, Care Coordinators (IDD) to oversee a team delivering member-centered care for individuals with intellectual and developmental disabilities. You will ensure timely, compliant care coordination and drive quality outcomes across the program.

The role emphasizes coaching staff, monitoring performance, and collaborating with care managers, leaders, and providers to optimize member experiences and service delivery.

Qualifications

• Bachelor's degree in a human service, social science, or health field.
• Five (5) years' experience in Care Management, with at least three (3) years related to IDD.
• Strong knowledge of IDD-related services, community resources, and regulatory requirements.
• Proficiency in documentation, reporting, and use of electronic health records or case management software.

Responsibilities

• Lead and supervise a team of care coordinators to ensure high-quality member-centered service delivery.
• Monitor care coordination activities, staff productivity, and caseload performance for timely completion of assessments and care plans.
• Ensure compliance with policies, contractual requirements, and state/federal regulations through oversight and audits.
• Provide coaching, performance feedback, training, and professional development to strengthen staff skills.
• Review dashboards and reports to identify trends and process improvements.
• Collaborate with providers, families, interdisciplinary teams, and community resources to support individualized care plans.
