Qualifications

Two (2) years+ working with individuals with intellectual developmental disabilities.
EHR proficiency and basic computer skills required.
Bachelor’s degree in social work, sociology, psychology, gerontology, or related field; RN or LPN credentials preferred.

Responsibilities

Serve as primary contact for members and authorized representatives.
Assess member needs and develop person-centered care plans.
Coordinate services across continuum and facilitate transitions between settings.
Monitor progress and adjust care plans to address changing needs.
Educate members and families on resources and advocate for care.
Maintain accurate documentation and ensure compliance with policies.
