Job summary

Advance Care Alliance New York is seeking a Care Manager to provide person-centered care management services. You will coordinate comprehensive assessments and develop Life Plans with an interdisciplinary team, focusing on meeting medical, behavioral health, and social needs. Travel up to 25% may be required to meet members and providers.

Qualifications

• Bachelor's degree (or RN license) with relevant experience, or master's with equivalent experience.
• Fluent understanding of community cultural traditions, norms, and practices of Members and families.
• Integrity and commitment to serving people with I/DD and their families.

Responsibilities

• Deliver person-centered care management services following regulatory standards.
• Complete comprehensive assessments and reassessments.
• Advocate for members to access needed services and reach goals.
• Develop Life Plans with an interdisciplinary team and member at the center.
• Cooperate with team to resolve conflicts in the planning process.
