Job summary

Advance Care Alliance New York seeks a Care Manager to coordinate services for individuals with intellectual and developmental disabilities. You will develop Life Plans, coordinate care with families and providers, and advocate for member needs to achieve meaningful outcomes.

The role requires travel up to 75% and a strong commitment to person-centered, rights-based care, with experience in health home and HCBS programs. 4-year degree or equivalent clinical experience is expected.

Qualifications

• Bachelor's degree (or RN license) with relevant experience; or master's with relevant experience.
• Fluent understanding of community cultural traditions and practices of members and families.
• Integrity and commitment to serving people with I/DD and families.
• Excellent interpersonal, public speaking, and written communication skills.
• Ability to work autonomously and demonstrate professionalism within a team.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete comprehensive assessment/reassessment processes.
• Identify gaps in service provision and make referrals; advocate for members.
• Develop, implement, and monitor Life Plans with an interdisciplinary team.
• Coordinate access to high-quality healthcare services and track follow-up actions.
