Qualifications

• Experience in human services or related field required.
• High school diploma or GED required.
• Operating knowledge of Microsoft Office applications and e‑mail.
• Maintains criminal and driving checks acceptable with State of Colorado and Cheyenne Village requirements.
• Equivalent combination of education and experience may be considered.

Responsibilities

• Ensures health and safety is monitored; follows safety procedures; reports unsafe conditions; works with team to address risks; administers medications as directed.
• Implements and documents Individual Service and Support Plans; supports Person‑Centered Planning; documents progress notes, attendance, and incident reports.
• Promotes participation in activities and personal interests; supports choice in all activities.
• Assists residents in developing relationships with community members and supports reciprocity.
• Participates as a team member to accomplish agency goals.
• Provides hands‑on daily living care including hygiene, housekeeping, shopping, meals, social/spiritual activities; assists with high support needs.
• Uses medical equipment such as Hoyer Lift, BP cuff, oxygen tanks, and pulse oximeter.
• Completes staff checklist daily; maintains residence appearance.
• Performs other duties as assigned.
