• Proficiency in documentation, reporting, and use of electronic health records or case management software.
• Proficient in Microsoft Office applications.

Responsibilities

• Lead and supervise a care management team to ensure high-quality, member-centered service delivery.
• Monitor care management activities, staff productivity, and caseload performance for timely completion of assessments and care plans.
• Ensure compliance with policies, contractual requirements, and state/federal regulations through oversight and audits.
• Provide ongoing coaching, performance feedback, training, and professional development to staff.
