Job summary

The Columbus Organization is seeking a Community Navigator to coordinate care for the IDD population in Newark, DE, with hybrid work and travel across Kent County. The role focuses on assessments, service planning, and crisis support to ensure continuous, high-quality care.

Ideal candidates have at least one year in health or human services, with relevant education or certification and strong communication skills.

Qualifications

• Minimum one year of experience in health or human services, including assessing individuals’ personal, health, employment, social, or financial needs.
• Associate’s degree in health or human services or a related certificate OR at least one year of interview/assessment experience.
• Experience contributing to client service plans and recommending treatment or eligibility.
• Knowledge of relevant laws, regulations, policies, and procedures and ability to apply them.
• Proficiency with electronic information systems for data entry, documentation, and reporting.
• Strong written communication skills.

Responsibilities

• Serve as liaison for participants, families, providers, and community partners for coordinated services.
• Conduct assessments and develop person-centered service plans aligned with goals and resources.
• Monitor and evaluate service delivery, addressing gaps and ensuring health, safety, and satisfaction.
• Provide casework, resource education, and crisis support.
• Maintain accurate electronic records and HIPAA/compliance.
• Build natural support networks and promote participant engagement.
• Conduct outreach to expand awareness of agency services.
