Job summary

Commonwealth of Pennsylvania seeks a compassionate Supports Coordinator in Honesdale, PA to help individuals access services and participate in their communities. You will develop and update Individual Support Plans, coordinate services, and engage with families and teams to review progress.

The role requires a relevant social welfare degree or equivalent experience, with pathways including internships or county casework.

Qualifications

• Six months of experience as a County Caseworker 1 or
• Successful completion of the County Social Casework Intern program or
• A bachelor's degree with a social welfare major or
• A bachelor's degree which includes 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences and one year of professional social casework experience in a public or private social service agency or
• Any equivalent combination of experience and training which includes 12 college credits in related social sciences.

Responsibilities

• Plan Development: Create and update Individual Support Plans reflecting each person’s needs
• Service Coordination: Connect individuals and families with supports and resources within their communities
• Community Engagement: Encourage individuals to participate in work and daily activities when appropriate
• Training Participation: Complete initial and ongoing training through supervision and peer shadowing
• Field Outreach: Travel to meet individuals and teams to review needs and progress

Education

Bachelor's degree with a social welfare major

Bachelor's degree plus 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences and one year of professional social casework experience

Six months of experience as a County Caseworker 1

Successful completion of County Social Casework Intern program

Any equivalent combination of experience and training with 12 college credits in related social sciences
