Job summary

Dominion Care is seeking an ID/DD Waiver Service Coordinator to oversee service delivery and coordinate care for individuals with intellectual and developmental disabilities. You will mentor staff, ensure documentation accuracy, and maintain compliance with Medicaid and CARF/QSR standards.

The role emphasizes leading a team, improving care plans, and coordinating with providers across Virginia, with competitive hourly pay and a robust benefits package.

Qualifications

• Bachelor's Degree from an accredited college or university in a field related to Human Services and one year of documented experience providing direct services with individuals with a diagnosis of an intellectual or other developmental disabilities.
• Equivalent Experience for Service Coordinator: three years of paid experience in directing, development and implementation, supervision and monitoring of staff.
• Valid driver's license.
• Leadership to mentor staff and ensure compliance with regulatory standards.

Responsibilities

• Oversee service plans, care coordination and staff supervision in the Waiver department.
• Complete quarterly progress summaries and home visits for individuals served.
• Perform monthly contacts and daily note audits to ensure Medicaid billing requirements are met.
• Serve as a point of contact for providers for service planning and reporting of progress/concerns.
• Assist in admission and discharge planning and participate in provider/community meetings.
• Ensure chart audits and documentation meet regulatory standards.
