Job summary

Qualifications

Responsibilities

• Provides direct nursing care to patients and families in a complex care area.
• Assesses patients, develops and implements individualized care plans, and documents accordingly.
• Delivers care in accordance with established policies and procedures and escalates as needed.
• Collaborates with the healthcare team to coordinate discharge planning and education for patients and families.
• Engages in performance improvement initiatives and supports staff development.
