Job summary

• Outcomes Identification/Planning - document planned interventions to meet patient needs.
• Implementation/Evaluation - evaluate and adjust care plans with evaluative data.
• Leadership - participate in process improvement for quality and safety.
• Operations - support daily department operations and coordinate care.
