Job summary

Sertoma Star Services is seeking a Personal Support Worker to provide care and support to individuals in Intermittent Community Integrated Living Arrangements (I-CILA). You will work within the Community Support Team to deliver person-centered services, with flexibility for days, evenings, and weekends as needed.

Responsibilities include implementing care plans, supporting daily living activities, documenting accurately, and ensuring a safe home environment.

Qualifications

• High School Diploma or GED recognized by the Illinois Department of Human Services.
• Demonstrated 8th grade reading level as evidenced by standardized testing.
• Valid Illinois driver's license with good driving record.
• Completion of all required DSP training within 120 days of hire and maintenance of required certifications.
• Minimum of one year experience working directly with individuals with developmental disabilities preferred.

Responsibilities

• Ensure compliance with DHS Rule 115 & 116, CARF standards, agency policy & procedure, program guidelines and licensing standards.
• Implement ISP and intervention plans and provide supervision as outlined in the plan.
• Attend required DHS and Sertoma Centre trainings and meetings; maintain competency.
• Communicate professionally and respectfully, protecting confidentiality of individuals served.
• Complete and submit all required documentation by deadlines (service notes, ISP, incident reports, etc.).
• Ensure a safe, healthy environment and proper storage of program items, medication, and food.
• Take direction from supervisors to address the needs and priorities of those served.
