Job summary

Sertoma Star Services is seeking an Intermittent CILA Service Facilitator in Illinois to support individuals with developmental disabilities in ICILA homes or in their own homes. The role focuses on daily living skills, monetary skills, community integration, learning, and medication management.

You will implement PCPs, document progress, and ensure services are delivered in a person-centered manner. Flexible hours, including some evenings and weekends, may be required to support participants.

  • Ensure services comply with DHS Rule 115/116, standards and agency policies.

Job description

Position Summary: The Intermittent CILA (ICILA) Service Facilitator provides for the basic care and training of individuals with developmental disabilities. This involves the areas of daily living skills development, monetary skill development, community integration, learning, and medication management. This position is responsible for the day-to-day implementation and facilitation of services to individuals residing in the ICILA homes and/or individuals residing in their home not controlled by the agency, ensuring services are delivered in a person-centered manner.

Essential Duties and Responsibilities
  • Responsible for the implementation of a Person-Centered Plan (PCP) and intervention plans and providing the level of supervision outlined in the plan.
  • Completes and submit all required documentation by assigned deadlines including, but not limited to, service notes, intervention plans, PCP’s, incident and accident reports, goal and behavior tracking, medication documentation, emergency drills, physical plan reviews, temperature logs, safety checklist, and housekeeping documentation as required.
  • Ensures a safe and healthy environment that fosters independence and self-determination. Ensure that all housekeeping tasks are completed, that program items, supplies, medication and food are stored and used as per guidelines, and that all safety issues are immediately brought to the attention of the immediate supervisor.
  • Responsible and accountable for the location and activities of all individuals in the home. Utilize natural environment for training opportunities, including activities of daily living. Prove the level of assistance requested or needed to individuals in completion of daily tasks/responsibilities/activities.
  • Takes direction from the individuals supported. Work with the Vice President of Community Living, Community Living Administrators, Residential Service Coordinators, to ensure that the needs and priorities of those served is effectively addressed.
  • Ensures delivery and/or coordination of all Residential services are in compliance with DHS Rule 115&116, CQL standards, agency mission, agency policy and procedure, program guidelines and best practice.
  • Actively participate as a member of the Community Support Team for each individual participating in the ICILA program.
  • Maintains flexible availability to support consumers in the ICILA program (i.e. days, evenings, and weekends as needed).
  • Uses sound business and customer service practices in providing support to internal and external customers.
  • Communicates in a professional, respectful and effective manner promoting an atmosphere that is responsive to the needs and desires of individuals served and reflects a commitment to the delivery of support in a person-centered manner.
  • Functions as advocate ensuring protection of an individual’s rights while providing a training element to empower persons with disabilities to understand and protect those rights.
  • Attends and participate in IDT meetings, when possible, to provide input relative to the care plan objectives, as related to the consumers’ health status.
  • Attends all agency mandated training within required timeframes
  • Meets requirements and maintain compliance of applicable licensing, funding, accreditation, and other state/federal regulatory agencies, including safety requirements and agency policies and procedures.
