Job summary

Sertoma Star Services is seeking an Intermittent CILA Service Facilitator in Illinois to support individuals with developmental disabilities in ICILA homes or in their own homes. The role focuses on daily living skills, monetary skills, community integration, learning, and medication management.

You will implement PCPs, document progress, and ensure services are delivered in a person-centered manner. Flexible hours, including some evenings and weekends, may be required to support participants.

Qualifications

• Implement Person-Centered Plan (PCP) and intervention plans as required.
• Complete and submit service notes, PCPs, incident reports, and related documentation on deadlines.
• Maintain a safe, clean, and organized living environment with proper storage of meds, supplies, and food.
• Provide support in daily living activities and empower individuals to exercise their rights.

Responsibilities

• Implement PCP and intervention plans and supervise as outlined.
• Submit all required documentation by deadlines (notes, PCPs, incidents, progress, etc).
• Maintain safety; ensure proper storage of household items, meds, and supplies.
• Assist with activities of daily living and community integration.
• Participate in IDT meetings when possible and coordinate with supervisors.
• Ensure services comply with DHS Rule 115/116, standards and agency policies.
