Job summary

Keystone Community Services is hiring a Waiver Case Manager Supervisor in Saint Paul, MN. The role leads a team delivering Home and Community Based Waiver services (AC/EW) across Ramsey and Hennepin counties, with a small caseload and program compliance responsibilities.

The position offers a hybrid schedule, three days in the office, and a hiring bonus after 30 and 90 days. Salary ranges from $55,000 to $60,000 annually with comprehensive benefits and paid holidays.

Qualifications

• Bachelor's degree required or related field.
• 2 years of Case Management experience.
• Understanding of professional boundaries and community resources.
• Knowledge of case management issues for serious mental illness or developmental disability.
• Experience with billable time/units.
• Valid Minnesota Driver's License and insurance for travel.

Responsibilities

• Provide leadership and supervision of Waiver Case Management staff.
• Assist with hiring, training, intern management and program development.
• Maintain a small caseload and ensure service delivery to Keystone standards.
• Oversee referrals, training materials, and program reporting.
• Coordinate with Ramsey County/ Hennepin County and DHS for audits and compliance.
• Provide back-up coverage for staff and related duties.
