Qualifications

• Bachelor’s degree in social work or a related field is required.
• One year of experience with developmental disabilities or related conditions, or training in that area.

Responsibilities

• Foster trusting relationships with individuals served, emphasizing empathy, effective communication, and compassion.
• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams to ensure seamless care delivery.
• Connect individuals with community resources to support independence and well-being.
• Ensure compliance with guidelines and meticulous documentation.
• Complete daily case noting of tasks.
• Collaborate to adjust care plans as needs evolve.
• Provide support and guidance to individuals and families for informed decisions.
• Travel to meet with clients for Annual and Semi-Annual meetings as needed.
