Job summary

Spurwink in Portland, ME is seeking a Case Manager to support transition-aged youth through the FYI program. This hybrid role combines field coordination with on-site documentation and client intake duties.

You will collaborate with providers to meet home, school, medical, and community needs, conduct psychosocial assessments, develop 90-day plans, and maintain monthly contact with families. BA required; driving license and comfort with multiple systems are needed.

Qualifications

• BA in related field required.
• Strong organization and communication skills.
• Savvy computer skills to manage multiple systems.
• Current driver's license and a clean driving record per Spurwink Driving policy.

Responsibilities

• Coordinate with providers to address home, school, medical, and community needs for each child.
• Complete intakes, psychosocial assessments, and other intake assessments; maintain weekly documentation.
• Make referrals and develop initial and 90-day treatment plans; provide minimum monthly contact with each family.
• Collaborate with providers to ensure needs are addressed and documentation is up to date.
