Job summary

Buckner Children and Family Services in Lubbock, TX seeks an FYI Case Manager to provide case management for foster youth aging out and alumni seeking self-sufficiency. You will coordinate education, housing, employment, and living skills, and connect participants with community resources and referrals.

Travel across a multi-county region, document activities per TWC grant guidelines, and collaborate with the FYI team and external agencies to expand services and opportunities for clients.

Qualifications

• Bachelor's degree in social services or related field required.
• Minimum 1 year of related casework experience required.
• Proficient ability to speak, read, and write English; Spanish preferred.

Responsibilities

• Assist participants with self-sufficiency plans covering education, training, employment, and living skills.
• Provide case management, review plans, case notes, and referrals.
• Help participants access housing and community resources and coordinate services.
• Develop relationships with institutions for coordinated services.
• Travel across a multi-county region to deliver services as needed.
• Complete documentation for the TWC grant per guidelines.
