Job summary

Buckner Children and Family Services in Lubbock, TX is seeking a FYI Case Manager to deliver case management for foster youth aging out and alumni seeking self-sufficiency. You will develop and monitor individualized plans, coordinate education and living skills training, and liaise with community resources.

Travel regionally to provide services, promote the FYI program, and collaborate with Buckner and partner agencies.

Qualifications

• Requires a bachelor’s degree in social services or a related field.
• Minimum 1 year of related casework experience.
• Proficient knowledge of community resources.
• Ability to speak, read, and write English; Spanish preferred.
• Must have valid driver’s license and be eligible for Buckner insurance.

Responsibilities

• Assist participants with self-sufficiency plans covering education, training, employment, and living skills.
• Provide and update individualized service plans, notes, and referrals.
• Help participants access housing, enroll in programs, and obtain community resources.
• Develop relationships with institutions to coordinate services for participants.
• Develop and deliver independent living skills curriculum; travel as needed.
