Job summary

Optum is seeking a Telephonic Case Manager RN in Georgia. The role combines outbound member outreach with education, advocacy and care coordination, aiming to improve health outcomes. Training is onsite, followed by a work-from-home arrangement with occasional on-site meetings in Sandy Springs.

Requires 3+ years RN experience (hospital or telephonic case management) and GA license; travel to the Sandy Springs office may be needed. Hybrid schedule with in-office days and 9am–9pm hours.

Qualifications

• Active, unrestricted RN license in the State of Georgia.
• Willingness to obtain multiple state/compact licensure within 60 days of hire.
• 3+ years RN experience in hospital/acute care or telephonic case management.
• Solid data entry skills and Windows navigation.
• Travel to the Sandy Springs (Atlanta), GA office based on business needs.
• Quiet workspace with access to secure high-speed internet at home.

Responsibilities

• Make outbound and take inbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to support self-management.
• Interact with Medical Directors on challenging cases.
• Coordinate care for members.
• Educate members on disease processes.
• Encourage healthy lifestyle changes.
• Document and track findings.
• Make post-discharge follow-up calls for needed services.
