Job summary

UnitedHealth Group’s Optum Care at Home is hiring a full-time Float Registered Nurse Case Manager to serve patients across Georgia. You will perform in-home visits, telephonic outreach, and collaborate with a multidisciplinary team to support medical, functional, and social needs.

Requirements include an unrestricted RN license in the state, 2+ years of nursing experience, ability to travel up to 75%, and strong communication.

Qualifications

• Current unrestricted RN licensure in state of practice.
• 2+ years of RN experience.
• Ability to travel up to 75% for field-based work.
• Experience in assessing medical, behavioral, social needs of patients.

Responsibilities

• Assess health status of members within licensure scope.
• Establish goals to meet identified health care needs.
• Plan, implement and evaluate responses to the plan of care.
• Collaborate with multidisciplinary team to address medical, functional, and social barriers to care.
• Coordinate care with PCPs and other health care professionals.
• Document in electronic medical record accurately.
• Provide counseling and education to patients and families.
