Job summary

Optum is seeking a Full-time Registered Nurse Case Manager for the Home & Community Care program in Georgia. The role involves in-home visits, telephonic outreach, and coordination with a multidisciplinary team to deliver comprehensive care.

You will assess patient needs, develop and implement care plans, and coordinate with PCPs and specialists while traveling within Georgia as required. A valid RN license and current state credentials are required.

Qualifications

• Current unrestricted RN license in state of practice.
• Minimum 2 years of RN experience.
• Experience with home health or complex patient needs.
• Able to travel up to 75% within Georgia.
• Valid driver's license is required.

Responsibilities

• Assess health status of members in their home or community setting.
• Plan, implement and evaluate care plans.
• Collaborate with multidisciplinary team to address medical, functional and social barriers.
• Coordinate referrals to community resources and services.
• Document care and patient education in the EMR.
• Support patients and families through transitions between care settings.
