Job summary

The position starts with onsite training, followed by a hybrid work arrangement requiring occasional in-person meetings at the Sandy Springs, Atlanta office. Requirements include an active Georgia RN license, 3+ years of RN experience, and ability to travel as needed.

Qualifications

• Active, unrestricted RN license in the State of Georgia.
• Willingness to obtain multiple state licensure within 60 days of hire.
• 3+ years RN hospital/acute care or telephonic case management experience.
• Computer proficiency with data entry and Windows (Word/Outlook/Excel).
• Must be willing to travel to Sandy Springs, GA office as needed.
• Designated quiet home workspace with secure high-speed internet.

Responsibilities

• Make outbound calls and take inbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans and coordinate care.
• Provide patient education to support self-management.
• Collaborate with Medical Directors on challenging cases.
• Coordinate care for members and document findings.
• Educate members on disease processes and healthy lifestyle changes.
• Document and track findings for continuity of care.
• Make post-discharge follow-up calls to ensure service access.
