Job summary

Optum is seeking a Full-time Float Registered Nurse Case Manager to support the Optum at Home program across Georgia. In this home-based role you will assess health status, develop and implement care plans, and coordinate with a multidisciplinary team to address medical and social needs.

Travel up to 75% is required, with documentation in EMR and ongoing patient education. Ideal candidates hold an unrestricted RN license, have at least 2 years of RN experience, and can collaborate with

Qualifications

Current unrestricted RN licensure in the state of practice.
2+ years of experience as a Registered Nurse.
Experience in assessing medical needs of patients with complex behavioral, social and/or functional needs.
Ability to work with diverse care teams in home settings.
Proficient in using computers and navigating the Internet.
Valid Driver's License and ability to travel up to 75% for field work.

Responsibilities

Assess the health status of members within licensure scope and model frequency.
Establish goals to meet identified health care needs.
Plan, implement and evaluate responses to the plan of care.
Collaborate with multidisciplinary team to address medical, functional, and social barriers to care.
Coordinate with mental health clinicians to bridge mental and physical health.
Consult with PCPs, specialists, or other health care professionals as appropriate.
Refer patients to community resources and services as needed.
Document thoroughly in the patient’s electronic medical record.
Provide counseling and education on health maintenance and follow-up.
Verify patient/family understanding of the plan of care and follow-up.
