Job summary

Hamilton Health Box is seeking a Nurse Practitioner to manage onsite employer clinics and rural health sites with a physician, in a hybrid model mixing virtual and in-person care.

The role emphasizes protocol-driven care, disease management, health coaching, and preventive screening, with data-driven population health efforts. Primary responsibilities include collaboration with clinicians, patient education, and care coordination across multiple facilities.

Qualifications

• Current NP license and board eligibility as applicable.
• BLS certification required.
• DEA registration or ability to obtain it.
• Minimum 2 years in primary care, family, internal, urgent care, or similar outpatient setting.
• Willingness to practice across state lines where permitted.
• Bilingual Spanish/English preferred.

Responsibilities

• Provide medical services with physician collaboration.
• Assess and manage patient care with diagnostic testing as needed.
• Develop and implement patient-management plans and protocols.
• Educate and counsel patients on health, growth, and daily living.
• Support telemedicine visits and on-site care coordination.
