Job summary

Hamilton Health Box is seeking a licensed Nurse Practitioner to manage 3-5 micro primary care clinics, delivering virtual and in-person care under physician supervision. You will focus on health coaching, disease management, preventive screening, and education, coordinating care across facilities, utilizing telemedicine, and supporting urgent care teams while adhering to NM APRN requirements.

This role also involves float duties and collaboration with care teams to ensure high-quality patient

Qualifications

• Graduate of a Nurse Practitioner program approved by the State of New Mexico.
• Master of Nursing required.
• Licensure as an NP in New Mexico with eligibility for full prescriptive authority.

Responsibilities

• Promotes and maintains patient health under physician supervision.
• Identifies patient care issues to improve physician efficiency.
• Recommends options and actions for patient care.
• Performs injections, immunizations, and wound management.
• Conducts health interviews and physical examinations; orders/interprets tests.
• Develops patient-management plans and educates patients.
• Adheres to infection-control policies and regulatory requirements.
• Utilizes telemedicine interface for virtual visits.
• Floats between facilities and supports urgent care APPs.
