Job summary

Hamilton Health Box seeks an Advanced Practice Provider to manage 3-5 primary care micro clinics, combining virtual and in-person care to deliver high-touch, low-cost healthcare for small and medium businesses.

The APP will work under physician supervision, developing patient-management plans, coaching on health maintenance, and coordinating across multiple sites to improve community health outcomes.

Qualifications

• Graduate of a Nurse Practitioner program approved by the State of New Mexico.
• Master of Nursing is required.
• Licensed to practice as an Acute Care Nurse Practitioner in NM or another state.

Responsibilities

• Promotes and maintains patient health under supervision of a physician.
• Identifies short- and long-term patient care issues to improve efficiency.
• Interprets diagnostic tests and documents patient care in records.
• Provides injections, immunizations, suturing, and wound care as needed.
• Uses telemedicine interface to conduct virtual visits and coordinate care.
