Qualifications

• Experience with individuals with disabilities is preferred.
• Bachelor's degree or equivalent plus related coursework/experience as described in job posting.

Responsibilities

• Develop and enter Individual Support Plans into the Home and Community Services Information System within required time frames, involving the individual and their team.
• Monitor health and safety of individuals on the assigned caseload.
• Develop and coordinate supports and services for individuals with intellectual disabilities or autism.
