Job summary

Qualifications

• Bachelor’s degree in Psychology, Social Work, Criminal Justice, Human Services or related field is required.
• Strong passion for supporting individuals with intellectual/developmental disabilities.
• Excellent organizational and documentation skills.
• Proficiency with Microsoft Office applications, calendaring tools, and electronic documentation systems.
• Valid driver’s license and willingness to travel within a 30–40 minute radius.

Responsibilities

• Develop and maintain individualized service plans with individuals, families, providers, and interdisciplinary teams.
• Manage a caseload of approximately 40-50 individuals and travel within the assigned territory for in-person meetings and monitoring.
• Coordinate, monitor, and evaluate authorized services for appropriate care and community support.
• Conduct routine monitoring visits and assessments of health, safety, well-being, and outcomes.
• Communicate with providers, families, and support teams to address concerns and ensure timely follow-up.
• Maintain detailed documentation, case notes, referrals, service records, and billing compliance.
