Job summary

Piper Companies is seeking a Supports Coordinator to join a mission-driven human services organization in Montgomery County, PA, to support individuals with ID and ASD. The role emphasizes advocacy, service coordination, documentation, and helping individuals access services.

You will manage a caseload, coordinate and monitor services, conduct visits in homes and community settings, and maintain communications with individuals, families, and providers.

Qualifications

• Bachelor's degree required for the role.
• Degree in Psychology, Social Work, Criminal Justice, Human Services, or related field preferred.
• Valid driver's license, reliable personal vehicle and automobile insurance required.
• Ability to travel locally throughout assigned territory.
• Strong written communication, organizational, and time management skills.
• Passion for supporting individuals with disabilities and willingness to learn through training.

Responsibilities

• Manage a caseload of individuals with ID and ASD.
• Coordinate, monitor, and advocate for services supporting health, safety, and quality of life.
• Conduct monitoring visits in homes, community settings, and day programs across the region.
• Connect individuals with services including transportation, residential supports, job coaching, and community programs.
• Maintain communication with individuals, families, and providers for effective service delivery.
• Complete detailed documentation including case notes, monitoring reports, and billing/compliance records.
