Job summary

North Country Community Mental Health in Petoskey, MI is seeking a Supports Coordinator Assistant for an entry-level, paraprofessional role that provides supports to individuals with developmental disabilities and/or mental illness under supervision. You will assist with plans of service and effective access to community resources.

The position is full-time with hybrid eligibility, offering starting wages of $20.11–$21.12 per hour and a benefits package including health/dental/vision options,

Qualifications

• Education: Minimum of high school diploma or equivalent required. Credit given for relevant life experience. Some college preferred, especially courses in social work, education, psychology, nursing, speech pathology, recreation, occupational therapy, physical therapy, human services or other related field.
• Experience: One-year experience in working with persons with developmental disabilities and/or mental illness is preferred but other combinations of experience will be considered
• Other: Lived experiences with behavioral health issues are valued.

Responsibilities

• Assist in development of Plans of Service for specific individuals, gathering data, completing forms, taking notes and writing up minutes.
• Link individuals with community resources; duties may include transportation to appointments and home visits.
• Assist in monthly/bi-annual progress reports and status reviews and gather data for these reports.
• Monitor and assure compliance with applicable laws, rules, policies and procedures in Plan of Service development and implementation.
• Assist individuals and guardians in applying for entitlement benefits, financial and medical assistance, residential support funding.
• Attend meetings as needed or requested by the Supports Coordinator or supervisor.
• Provide transportation to appointments, advocate with providers, and work flexible hours as needed.
• Gather information for psychosocial assessments and support clinical planning.
