Network180 is seeking a Supports Coordinator to deliver person-centered planning and ongoing linkage, coordination and monitoring of services for clients in Kent County. This full-time, hybrid role combines remote work with field meetings in the community.

The position requires a Bachelor’s degree in human services and relevant experience with intellectual/developmental disabilities, strong organization, listening and communication skills, and a valid driver’s license.

Qualifications

• Bachelor's degree in human services or related field is required.
• Experience or fieldwork with disabilities or developmental conditions is valued.
• Demonstrated ability to plan and coordinate person-centered services.

Responsibilities

• Develop and monitor an Individual Plan of Service through person-centered planning.
• Provide ongoing linking, coordination, and monitoring of services for clients.
