Job summary

North Country Community Mental Health is seeking a dedicated Supports Coordinator for its Gaylord, MI location. This full-time, non-union role focuses on service coordination for individuals with severe mental illness or intellectual/developmental disabilities.

The position requires a bachelor’s in social work or related field and eligibility as QIDP or QMHP, with strong computer skills and a commitment to recovery-oriented care.

Qualifications

• Bachelor’s degree in Social Work or a related human services field is required; Michigan licensure/limited licensure preferred.
• One year of experience delivering services to IDD or SPMI populations; QMHP or QIDP eligibility preferred.
• Proficient computer skills including Office 365.

Responsibilities

• Develop, review, and modify a Person-Centered Plan of Service in collaboration with clients and guardians.
• Act as a link between client care providers to ensure coordinated services and information flow.
• Monitor service quality, health and safety; ensure plans are implemented as authorized.
• Coordinate resources and assist during crisis and transportation needs.
• Maintain timely, accurate documentation in the clinical record, compliant with Medicaid requirements.
