Job summary

Green Key Resources is seeking a telephonic care management professional to serve an assigned population. You will be the sole communicator between member and the plan, coordinating benefits and reviewing person-centered service plans with members and families.

The role requires an MSW with 2-3 years of care management experience; LMSW is preferred. You will maintain monthly contact and follow up, addressing identified items and participating in weekly team meetings.

Qualifications

• Master of Social Work (MSW) required.
• LMSW licensure preferred.

• Perform telephonic care management services to assigned population.
