Dartmouth-Hitchcock Health is seeking a Care Management professional with a Master’s Degree in Social Work (MSW). The role provides psychosocial assessments, counseling and coordination of community resources for patients and families.

40 hours per week; strong organizational, negotiation, and communication skills are essential. Sign-on bonus eligible: $5,000. Must be able to handle complex cases and collaborate with multiple agencies to support patients and families.

Aufgaben

• Provides complete psychosocial assessments of patients and families.
• Provides counseling to individuals and families to help solve social and emotional problems.
• Coordinates community resources to address/meet identified needs.
• Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
• Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
• Documents objective, concise and pertinent clinical data in the medical record.
• Perform other duties as assigned.
