Job summary

Utah Case Management is seeking compassionate Social Work Case Managers to support elderly and mental health clients through Utah Medicaid’s New Choices Waiver program. This flexible hybrid role blends in-person client work with remote documentation, estimated at about 20–40 hours per week.

You will coordinate services, visit clients in homes or facilities, and advocate to ensure services meet needs, while managing your caseload and documentation independently.

Qualifications

• Utah resident living in Saint George, Utah.
• Licensed in Utah as SSW/CSW/LCSW or RN.
• Able to travel locally for monthly client visits.
• Availability for in-person visits during normal business hours.
• Independent while collaborating with a team.
• Strong communication and organizational skills.
• Detail-oriented and able to maintain documentation on time.
• Self-motivated with own schedule and caseload.
• Able to build and maintain relationships with clients and providers.
• Must have reliable access to own computer/device.

Responsibilities

• Coordinate services for elderly and mental health clients in the NCW program.
• Maintain monthly face-to-face contact with clients.
• Visit clients at homes, assisted living, or long-term care facilities.
• Assess needs and coordinate services and resources.
• Communicate with clients, families, providers, facilities, and team.
• Advocate for clients and ensure services meet needs.
• Complete and maintain required documentation.
