Job summary

Utah Case Management is seeking a Registered Nurse to join our case management team in a dual Nurse and Social Work role. The position is hybrid with remote documentation and in-person client visits, located in the Saint George, UT area.

You will coordinate medical services and care for clients under Utah’s Medicaid New Choices Waiver program. The role requires nursing assessments, collaboration with a social worker, and timely documentation and care planning.

Qualifications

• Current Registered Nurse license in Utah.
• Utah resident living in St. George, UT.
• Ability to create and maintain rapport with clients.
• Detail oriented and self-motivated.
• Good communication and organizational skills.

Responsibilities

• Nurse responsibilities: Conduct nursing assessments (MDSHC, LOC, Medication Management Review); coordinate with social worker on healthcare decisions; conduct health status screenings after inpatient care; complete memory care documentation for memory care approvals; participate in required nursing trainings.
