Job summary

Utah Case Management is seeking a Registered Nurse to join the dual Nurse Case Manager team in the St. George, Utah area. The role combines nursing and social work duties with a hybrid schedule that includes remote documentation and in-person client visits.

Responsibilities span nursing assessments, teamwork with a social worker, and care coordination through Medicaid Waiver programs. A valid Utah RN license, strong rapport-building, and organization are essential.

Qualifications

• Utah resident living in St. George, UT.
• Current Registered Nurse license in Utah.
• Strong communication and organizational skills.
• Detail oriented and self-motivated.

Responsibilities

• Perform nursing assessments (MDSHC, LOC, medication management) within required timeframes.
• Collaborate with a social worker on healthcare decisions or client needs.
• Complete health status screenings after hospitalization or health declines.
• For memory care referrals, complete memory care checklist and necessary documentation.
• Attend required nursing trainings and other trainings as required.
• Coordinate services and care plans across nursing and social work aspects.
