Job summary

Utah Case Management in Salt Lake City is seeking compassionate Social Work Case Managers to support elderly and mental health clients through Utah Medicaid’s New Choices Waiver (NCW) program. This flexible hybrid role combines in-person work with remote documentation, estimated at 20-40 hours per week.

You’ll coordinate services, maintain monthly face-to-face contact, visit clients at homes or facilities, and document accurately.

Qualifications

• Utah resident currently residing in SALT LAKE COUNTY.
• Current Utah SSW, CSW, LCSW, or RN license.
• Ability to travel locally for monthly client visits.
• Availability for in-person client visits during normal business hours.
• Ability to work independently while collaborating with a team.
• Strong communication and organizational skills.
• Detail-oriented with accurate documentation.
• Self-motivated with strong time management.
• Able to build positive relationships with clients, families, facilities and providers.
• Reliable access to own computer/device for remote work.

Responsibilities

• Maintain monthly face-to-face contact with assigned clients.
• Visit clients in their homes, assisted living communities, or long-term care facilities.
• Assess client needs and coordinate appropriate services and resources.
• Communicate regularly with clients, families, service providers, facilities, and care team members.
• Advocate for clients to ensure services meet needs.
• Complete, update, submit, and maintain required client documentation.
• Communicate with team to coordinate and support client services.
• Attend staff meetings and trainings.
• Manage caseload independently while meeting documentation and scheduling expectations.
• Travel locally to complete monthly client visits.
