Job summary

Utah Case Management is seeking a Social Work Case Manager to support elderly and mental health clients through Utah Medicaid’s New Choices Waiver program. This flexible hybrid role combines in-person client work with remote documentation, and involves meaningful field visits across Price, Utah.

You will build relationships, coordinate services, and navigate resources to meet client needs. The position estimates 20–40 hours weekly with a variable caseload and billable, Medicaid-reimbursable work.

Qualifications

• Utah resident currently in Price, Utah.
• Current Utah SSW, CSW, LCSW or RN license.
• Ability to travel locally for monthly client visits.
• Availability for in-person visits during normal business hours.
• Self-motivated with strong communication and documentation skills.
• Detail-oriented and able to manage caseload and documentation.

Responsibilities

• Coordinate services for elderly and mental health clients in the NCW program.
• Maintain monthly face-to-face contact with assigned clients.
• Visit clients in homes, assisted living, or long-term care facilities.
• Communicate with clients, families, providers and care team.
• Advocate for client needs and ensure services meet requirements.
• Complete and maintain required client documentation.
• Travel locally for monthly client visits and engage in team meetings and trainings.
