Job summary

UPMC Western Behavioral Health is seeking Service Coordinators in the Pittsburgh area. This hybrid role focuses on connecting clients with community resources and coordinating services, with a schedule that includes four 10-hour shifts after six months and occasional evenings.

Qualifications

• Bachelor's degree in Human Services or related field.
• One year of related experience preferred.
• CCM training within 30 days of hire and annually thereafter.

Responsibilities

• Advocate for individuals to access necessary services.
• Ensure monthly contact and provide comprehensive service coordination.
• Facilitate service planning meetings and communicate expectations.
