Qualifikationen

• Bachelor's degree in social work, psychology, or related field; practicum preferred OR at least 3 years of experience in social services or healthcare.
• Experience in case management and care coordination for adults with complex medical, behavioral, and social needs.
• Ability to perform in-home assessments and visits in a community-based setting.
• Strong communication and relationship-building skills with internal teams and providers.
• Commitment to confidentiality and HIPAA compliance.

Aufgaben

• Build relationships with internal teams and partner providers to coordinate medical, behavioral, and social services.
• Manage an active caseload based on state ratios and acuity; track progress and ensure OPS reporting deadlines.
• Prepare reports and update service plans, ensuring timely, compliant assessments.
• Lead Person-Centered Service Planning (PCSP) and assist Members in obtaining HCBS services.
• Coordinate LTSS and Medical Assistance services, navigate non-Medicaid supports, and monitor LTSS needs.
• Educate Members about LTSS options, DHS Fair Hearing rights, and facilitate access to services when needed.
• Gather Member preferences and goals to inform PCSP and reevaluation of care levels.
• Maintain LTSS eligibility activities and explore coverage through MA, Medicare, or private insurance.
• Coordinate with multidisciplinary teams to ensure seamless care and protect member health and privacy.
• Work 75% in the field and to work independently in a virtual setting.
• Support Providers in identifying qualified services and maintaining compliance with regulations.
