Job summary

UPMC Community Care Behavioral Health is seeking a Licensed Professional Care Manager for Allegheny County. The role offers a hybrid work structure with some remote time and travel across the community, with occasional in-office presence in downtown Pittsburgh.

Qualifications

• Licensed professional in good standing with behavioral health licensure.
• Experience coordinating care for high-need populations.
• Strong written and verbal communication skills.

Responsibilities

• Makes authorization determinations for medically necessary services within licensure.
• Coordinates care across clinical treatment, case management and community resources.
• Encourages coordination of care with primary care physicians and other service providers.
• Monitors and evaluates treatment outcomes and adjusts plans as needed.
• Assists members with transitions into or out of the county.
• Demonstrates excellent clinical, written and oral communication skills.
• Develops outreach plans for members lacking regular contact.
• Implements clinical interventions to improve outcomes.
• Collaborates with providers, state/federal agencies, and community groups.
• Maintains daily logs, reporting and weekly trend analyses.
