Job summary

The Service Coordinator will assess, plan, implement, coordinate, and monitor services to promote safe, independent living and high-quality outcomes for adults with complex needs, across medical, behavioral, and social domains.

Qualifications

• Strong background in case management and care coordination.
• Experience conducting in-home assessments and visits.
• Ability to work effectively with adult populations with complex medical, behavioral, and social needs.

Responsibilities

• Establish relationships with internal teams and partner providers to coordinate resources.
• Manage an active caseload and track participant progress for regulatory reporting.
• Prepare reports and update service plans, ensuring timely comprehensive assessments.
