Role Description The Service Coordinator is a full-time, hybrid role based in Yolo, CA, with some flexibility for work from home. The role involves managing a caseload of individuals and families, conducting intake and assessment, and developing person-centered service plans in accordance with applicable regulations and ACRC policies. The Service Coordinator coordinates services among community providers, schools, healthcare professionals, and families, ensuring timely access to supports and monitoring progress toward goals. Daily responsibilities include maintaining accurate and timely documentation, participating in interdisciplinary meetings, responding to inquiries, and supporting individuals and families in navigating resources and advocacy. The role also involves regular communication by phone, email, and in person, as well as occasional community outreach and collaboration with other team members.
