Job summary

Alta California Regional Center (ACRC) is seeking a Service Coordinator to provide case management and advocacy for children and adults with developmental disabilities. The role is based in the Yuba City office and involves IPP development, monitoring, and collaboration with families and community agencies.

Applicants should have a Bachelor’s or Master’s degree in Social Work or a related field, excellent communication skills, and strong organizational abilities.

• Excellent written, verbal, and interpersonal communications skills.
• Strong time management and organizational abilities.
• One year of professional experience in social work or related field preferred.

Responsibilities

• Provide case management and advocacy for individuals with developmental disabilities.
• Develop and monitor the Individual Program Plan (IPP) and revisions as needed.
• Assist clients and families in obtaining supports and services; advocate with community agencies.
• Consult with vendors, process applications and referrals; complete required forms and reports.
• Participate in on-call duties and assist coworkers with special projects and coverage.
