Job summary

Alta California Regional Center (ACRC) is seeking a Service Coordinator to provide case management and advocacy for individuals with developmental disabilities. This role is based in the Yuba City office and focuses on children ages 6-17, coordinating IPPs and supporting families in accessing services.

The position requires a Bachelor’s or Master’s degree in Social Work or a related field, plus strong communication and organizational skills.

Qualifications

• Bachelor’s or Master’s degree in Social Work or a related field.
• Excellent written, verbal, and interpersonal communication skills.
• Strong time management and organizational skills.

Responsibilities

• Provide case management and advocacy for individuals with developmental disabilities.
• Develop, monitor, and revise the Individual Program Plan (IPP).
• Assist clients and families in acquiring and maintaining supports and services.
• Advocate for clients with community agencies; consult with vendors, applications, and referrals.
