Job summary

All Abilities, Inc., a subsidiary of Lifesteps.inc, seeks a Service Coordinator to locate, coordinate, and monitor supports for adults with disabilities in Western PA. The role requires strong people skills and excellent customer service in a hybrid setting with home visits and client travel.

The SC will travel throughout Allegheny, Westmoreland, Fayette, Indiana, Armstrong, Butler and Beaver counties. Mileage reimbursement is provided; many cases are in Allegheny/Westmoreland counties.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Special Education, Sociology, Human Services, or related field, OR RN.
• Minimum 3 years of experience in social services or healthcare setting.
• Must use personal vehicle for travel with valid license and auto insurance; must obtain ACT 33/34 and FBI clearances.

Responsibilities

• Locate, coordinate, and monitor supports and services for adults with disabilities.
• Serve as initial contact for clients and build relationships with partners and providers.
