Job summary

IU Health is seeking an RN Care Manager to join the Kidney Care team. The role follows a Monday–Friday day shift with hybrid remote options and collaboration with other Care Managers across multiple nephrology offices in IU Health North, West, Fishers, University and Methodist.

You will advocate for patients, coordinate care with a multidisciplinary team, and employ patient-centered interventions to improve health outcomes and utilization metrics.

Qualifications

• Requires an active Registered Nurse (RN) license in Indiana or NLC.
• ASN required; BSN within 5 years of hire.
• Certification in Care Management preferred.
• 3-5 years of relevant experience required.
• Kidney Care experience strongly preferred.
• Experience in Utilization Management, Population Health, or Case Management preferred.
• Knowledge of Medicare/Medicaid, insurance and regulatory guidelines preferred.
• Strong clinical assessment skills preferred.

Responsibilities

• Provide exceptional care management services to Kidney Care populations with a multidisciplinary team.
• Advocate, guide, and support patients, families, and caregivers in care management programs.
• Implement patient-centered interventions and measure outcomes against clinical quality and utilization metrics.
• Contribute to broader population health objectives through continuous improvement.

Education

Associate's Degree in Nursing

BSN within 5 years

Active RN license in Indiana or NLC RN license
