Job summary

Indiana University Health is seeking an RN Care Manager to join the Kidney Care team. The role follows a Monday–Friday day shift with a hybrid remote option and will collaborate with another Care Manager to support Nephrology offices at IU North, West, Fishers, University and Methodist.

The position emphasizes patient-centered care, strong collaboration with multidisciplinary teams, and adherence to ethical practice and regulatory guidelines to drive quality outcomes.

Qualifications

• RN licensure in Indiana or NLC licensed.
• Care management certification preferred.
• 3–5 years of relevant experience.
• Kidney Care experience preferred.

Responsibilities

• Provide care management services for Kidney Care populations with a multidisciplinary team.
• Advocate, guide, and resource for patients, families and caregivers.
• Engage patients in appropriate care management programs with patient-centered interventions.
• Improve health outcomes and contribute to population health objectives.
